All services require a free scheduled intake and assessment process (30 minutes) which can be completed by phone or in person.

The initiation of services require a signed Service Agreement by the client or Medical Power of Attorney, in addition to the Durable Power of Attorney when applicable. If there is no MPOA or DPOA established then an attorney in-fact must be identified.

For clients an families currently on, or about to transition to, hospice services a Release of Information is strongly encouraged in addition to the signed Service Agreement for coordination of compassionate death doula care.
